{"id":1729,"date":"2020-06-22T12:03:14","date_gmt":"2020-06-22T16:03:14","guid":{"rendered":"https:\/\/dan.devhammer.com\/?post_type=dan_diving_incidents&#038;p=1729"},"modified":"2023-06-30T11:20:25","modified_gmt":"2023-06-30T15:20:25","slug":"immersion-pulmonary-edema-ipe-in-a-new-healthy-diver","status":"publish","type":"dan_diving_incidents","link":"https:\/\/world.dan.org\/fr\/safety-prevention\/diver-safety\/case-summaries\/immersion-pulmonary-edema-ipe-in-a-new-healthy-diver\/","title":{"rendered":"L'\u0153d\u00e8me pulmonaire d'Immersion (EPI) chez un nouveau plongeur en bonne sant\u00e9"},"content":{"rendered":"<p>Un plongeur est secouru apr\u00e8s avoir \u00e9t\u00e9 essouffl\u00e9 et avoir crach\u00e9 du sang alors qu'il effectuait des exercices de surface pour l'obtention de son brevet de plong\u00e9e.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">R\u00e9cit rapport\u00e9<\/h2>\n\n\n\n<p>Un plongeur de 56 ans participait \u00e0 une plong\u00e9e de la rive en vue de l'obtention de son brevet de plongeur d\u00e9butant Il n'avait pas d'ant\u00e9c\u00e9dents m\u00e9dicaux significatifs, \u00e0 l'exception d'une hypertension limite qui \u00e9tait r\u00e9guli\u00e8rement surveill\u00e9e, et il gardait une bonne condition physique. La temp\u00e9rature de l'eau \u00e9tait de 18\u00b0 C (65 \u00b0F) et il portait une combinaison de 7 mm, des gants et une cagoule. Le plongeur et son groupe ont nag\u00e9 environ 46 m\u00e8tres (50 yards) en alternant d\u00e9tendeur et tuba. En arrivant \u00e0 la bou\u00e9e de surface, il a ressenti une dyspn\u00e9e (essoufflement) et une fatigue importantes. Le plongeur en a inform\u00e9 le divemaster qui l'a remorqu\u00e9 jusqu'au rivage. Pendant le remorquage, le plongeur a d\u00e9velopp\u00e9 une h\u00e9moptysie (toux de sang) et des cr\u00e9pitants respiratoires audibles. Environ dix minutes apr\u00e8s \u00eatre sorti de l'eau, ses sympt\u00f4mes ont commenc\u00e9 \u00e0 s'am\u00e9liorer. Le plongeur s'est repos\u00e9 pendant deux heures avant de conduire jusqu\u2019\u00e0 chez lui. La toux et l'h\u00e9moptysie ont persist\u00e9 pendant six heures suppl\u00e9mentaires et les cr\u00e9pitants ont persist\u00e9 jusqu'au lendemain. La douleur pulmonaire a persist\u00e9 pendant environ une semaine. Le suivi m\u00e9dical n'a r\u00e9v\u00e9l\u00e9 aucune anomalie cardio-pulmonaire et un \u0153d\u00e8me pulmonaire d'immersion a \u00e9t\u00e9 suspect\u00e9.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Commentaire<\/h2>\n\n\n\n<p>L'\u0153d\u00e8me pulmonaire d'immersion (EPI), ou \u0153d\u00e8me pulmonaire induit par la nage, est une affection qui se manifeste par une dyspn\u00e9e, une h\u00e9moptysie et une d\u00e9tresse respiratoire lors d'une activit\u00e9 dans l'eau. Des cas ont \u00e9t\u00e9 signal\u00e9s lors de plong\u00e9e sous-marine, de triathlons, de natation militaire et de plong\u00e9e en apn\u00e9e.<\/p>\n\n\n\n<p>Le m\u00e9canisme de l'EPI est multifactoriel. L'immersion entra\u00eene une redistribution du sang de la p\u00e9riph\u00e9rie vers le c\u0153ur, ce qui se traduit par une augmentation du volume sanguin thoracique. Avec une augmentation de la r\u00e9sistance respiratoire, les pressions dans le syst\u00e8me vasculaire pulmonaire augmentent. Chez les personnes sensibles, les pressions art\u00e9rielles et capillaires pulmonaires peuvent devenir trop \u00e9lev\u00e9es et entra\u00eener une perte d'int\u00e9grit\u00e9 de la barri\u00e8re h\u00e9mato-gazeuse (d\u00e9faillance du stress capillaire pulmonaire). Cela peut entra\u00eener une fuite de liquide hors de la vascularisation pulmonaire et dans les poumons.1<\/p>\n\n\n\n<p>Dans ce cas, les facteurs contributifs possibles sont l'eau froide, l'augmentation du volume sanguin, l'exercice et l'augmentation de la r\u00e9sistance respiratoire due au d\u00e9tendeur et au tuba. De plus, l'hypertension est un facteur connu de susceptibilit\u00e9 et pourrait avoir contribu\u00e9 dans ce cas. Il a \u00e9t\u00e9 propos\u00e9 qu'une combinaison \u00e9tanche puisse \u00eatre une variable aggravante, mais cela reste \u00e0 prouver.<\/p>\n\n\n\n<p>L'apparition et la gravit\u00e9 de l'EPI peuvent varier de l\u00e9g\u00e8re \u00e0 mortelle. Bien que les cas disparaissent g\u00e9n\u00e9ralement dans les 24 \u00e0 48 heures, des d\u00e9c\u00e8s ont \u00e9t\u00e9 signal\u00e9s. Les sympt\u00f4mes s'aggravent avec une exposition continue, de sorte qu'une reconnaissance pr\u00e9coce et une sortie de l'eau font partie int\u00e9grante de la gestion de l'EPI. Dans ce cas, le plongeur a eu la sagesse de signaler rapidement les sympt\u00f4mes. Lorsqu'on lui a demand\u00e9 s'il pouvait nager seul jusqu'au rivage, le plongeur a demand\u00e9 de l'aide. Si le plongeur avait ignor\u00e9 les sympt\u00f4mes en les consid\u00e9rant comme une fatigue due \u00e0 l'effort ou s'il avait tent\u00e9 de regagner le rivage \u00e0 la nage seul, le r\u00e9sultat aurait pu \u00eatre beaucoup moins favorable. Apr\u00e8s sortir de l'eau, il convient d'administrer de l'oxyg\u00e8ne si possible et de consulter un m\u00e9decin. Comme de nombreuses affections peuvent se pr\u00e9senter de la m\u00eame mani\u00e8re, par exemple un accident cardiaque aigu, une \u00e9valuation appropri\u00e9e est justifi\u00e9e, de m\u00eame qu'une discussion sur la reprise de l'activit\u00e9, car l'EPI peut parfois r\u00e9appara\u00eetre.<\/p>\n\n\n\n<p><em>-Stefanie D.
